Reference Requests and Circulation Procedures

You may request information in person, by telephone, letter, fax or online Department Contact Form.

To expedite your request, we recommend you submit a completed RMD Form 4, Reference Request/Check Out Form, to State Records Center staff. 

Records or documents must be viewed at the State Records Center.  Upon request, Records Center staff will provide copies of materials for a fee.  For more information, please see our current fee schedule.  

Records circulate only to the originating agency of the records. 

You will be notified if access to records is restricted or requests cannot be completed.
