Records Management Forms
for State Agencies

Forms are available in both the Adobe Acrobat format (PDF) and Rich Text Format (RTF). You can download the free Adobe Acrobat reader if you don't already have it installed for your browser software. The RTF format files can be opened by Microsoft Word� and most other word processing programs. Some browsers will also open RTF files, but it may take a few moments.

Right-click on the file format you want under the form number you wish to view or download. Choose "Save As..." to save the form to your computer, or "Open" to view it with your browser.

You MUST send in a paper copy of the form that has been properly signed.
